54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today. Inspector observed 5 children (12-points) directly supervised by 2 caregivers. Ratio and supervision were in compliance. A sample of 2 children's records, and 2 caregiver records were reviewed. The physical plant, and emergency procedures were also reviewed. No medications were reported on site. The children were engaged in various activities, to include: free play outside, helping pick up sticks and the babies taking a rest under the sun. The interior of the family day home was found to be clean, organized, and well supplied with toys and equipment for the children. Areas of non-compliance are identified in this report. The exit interview was conducted with the Provider. Inspection conducted from 10:30am -11:30 am. Standard #: 22VAC40-111-60-B Description: Based upon record review, required immunization record was not on file. Evidence: Child A did not have an immunization record the first date in attendance (7/26/2016) as required. Plan of Correction: Provider has been in contact with parent regarding the immunization record. The child has only been in care two times since 7/26/2016 and parent will pick up form from doctor. Doctor will not fax immunization record to parent.
Standard #: 22VAC40-111-240-A Description: Based upon observation, equipment was found to be in need of repair. Evidence: See saw in backyard play area had areas of rust and was missing handle covers. Plan of Correction: Provider will fix or remove see saw from play area.
