Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and documentation of new child who started today.

Comments:
The following inspection took place between the hours of 11 am through 1 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located near the town of Aldie, Virginia. At the start of this unannounced renewal inspection there were a total of 8 children (21 points) with 2 caregivers. A total of 3 new children?s files and 4 staff/household member?s files were reviewed. The children were engaged in the following: computers, story time, music, diaper changing, bathroom breaks, hand washing, freeplay, toy play, games, nap time, and other organized activities. Violations:
Standard #: 22VAC40-111-210-A
Description: Based on review, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Evidence - 1. On the date of inspection (08/16/2016), Member #1 had obtained 12.5 hours of training for the current annual training year. 2. Member #2 had obtained 10.5 hours of training for the current annual training year.

Plan of Correction: Both Members are currently in the process of completing this training.

Standard #: 22VAC40-111-500-C
Description: Based on observation, the outdoor playground equipment, in some areas, is over a hard surface. Evidence - 1. On the date of inspection (08/16/2016), the swings of the outdoor playground equipment are no longer located over grass. 2. Due to continuous use, ruts have worn away the grass and have exposed the dirt ground underneath.

Plan of Correction: Rubberized mats will now be placed into to all ruts; covering up the exposed ground.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review, 3 of 4 staff/household member's files reviewed indicated that it has been more than three years since the dates of the most recent central registry (CPS) finding. Evidence - 1. On the date of inspection (08/16/2016), the following members had expired CPS findings on file at the Family Day Home (FDH): Member #1, Member #3, and Member #4. 2. The date of 07/20/2013 was the same date observed on three CPS findings for the applicable FDH Members listed above.

Plan of Correction: New requests for updated findings were sent in July 2016.
