An unannounced monitoring inspection was completed on 8/22/2016. The inspector arrived at 8:35 A.M. and departed at 11:15 A.M. Seven children were in care at 18 points with the licensee and one assistant caregiver. Three caregiver records were reviewed. Five children's records were reviewed. A physical plant walk-through was done. Discussion: subsidy requirements, standards, and 10 hours in required trainings.

Standard #: 22VAC40-111-750-A
Description: Based on observation, the licensee failed to ensure that that topical skin products were labeled with the child's name. Evidence: 1. There were three diaper ointments that were not labeled with the child's name.

Plan of Correction: During the inspection the licensee labeled the diaper ointments and will monitor all topical skin products.

Standard #: 22VAC40-111-780-B
Description: Based on review of five children's records, the licensee failed to ensure that all records were complete. Evidence: 1. The inspector found that child # 4 was not updated annually. Most recent update was on 3/24/2014.

Plan of Correction: The licensee will have the parent to update the record as soon as possible. In future all information will be updated annually.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the licensee failed to ensure that the training table was non absorbent. Evidence: 1. The inspector found that the changing pad was torn in two places which made the changing pad non absorbent.

Plan of Correction: The licensee will purchase a new pad and will monitor the areas.
