Today an unannounced renewal inspection was conducted. This inspection started at 12:30pm and ended at 1:45pm. The provider and two assistants were caring for 9 children, totaling 22 points. The children were observed being diapered, washing their hands, watching TV, listening to music, and napping. This inspector reviewed children and staff files, household member files, the physical plant, emergency documentation and procedures, and other care components. Standard #: 22VAC40-111-120-B Description: Based on record review, 1 of 3 staff files did not have 2 references. 1. Staff #2 had 1 of 2 references in their file. Plan of Correction: I will get the other reference.
Standard #: 22VAC40-111-60-B Description: Based on record review, 1 of 1 children's files were incomplete. Evidence: 1. Child #1 was missing information as to if the child had an allergies. Plan of Correction: I will get missing information.
Standard #: 22VAC40-111-80-C Description: Based on record review, 1 of 1 children's files did not adequately document the method of verifying the child's age. Evidence. 1. Child#1's file did not document the person viewing the child's birth certificate or the date that it was viewed. Plan of Correction: The provider provided the missing information, during the inspection.
Standard #: 22VAC40-111-180-A Description: Based on record review, 1 of 3 staff members did not have a current Report of Tuberculosis Screening documentation. Evidence: 1. Staff #1's Tuberculosis documentation expired on 07/21/16. Plan of Correction: I will have staff complete TB screening.
Standard #: 22VAC40-111-320-A Description: Based on direct observation, a small appliance was plugged in while not in use. Evidence: 1. A small coffee maker was plugged in , while not in use, in the kitchen. Plan of Correction: The appliance was unplugged, during the inspection.
Standard #: 22VAC40-111-320-B Description: Based on direct observation, there were two unused electrical outlets that were not covered. Evidence: 1. In the area where the children primarily are cared for, there was one uncovered outlet to the left of the window on the left side wall. 2. There was one uncovered outlet, behind an infant crib, on the upstairs level. Plan of Correction: This outlet was covered, during the inspection.
Standard #: 22VAC40-111-320-G Description: Based on record review, the wood burning fire place had not been annually inspected. Evidence: 1. The last inspection was completed in 2013. Plan of Correction: I will have a chimney inspection.
