Makes sure you are sounding the smoke detector when you are doing monthly escape drills, blowing a whistle is not sufficient. Discussed repeat violations and the need to improve compliance.
An unannounced renewal study was conducted today with the provider, who was present with four napping preschoolers for a total of 9 points. For lunch, the children were provided chicken rice with mixed vegetables, apples and milk. Four children and household member's files were reviewed today. The shelter in place drill was conducted on 4/4/16. Arrival time: 1:35 p.m. Departure time: 3:15 p.m.
Standard #: 22VAC40-111-60-B Description: Based on a review of four children's files, it was determined that one child did not have all the required information in the file. Evidence: Child A enrolled on 12/7/14 and there was no second emergency contact on file nor was their a insurance policy number listed. Plan of Correction: I will get this information from the parent.
Standard #: 22VAC40-111-180-A Description: Based on a review of four household member's files, it was determined that one file was missing an up-to-date tuberculosis test/screening every two years. Evidence: Person A's tuberculosis test was dated 4/4/14. Plan of Correction: Person A will get an up to date tuberculosis test/screening.
Standard #: 22VAC40-111-210-A Description: Based on a review of files, it was determined that one person did not have 16 hours of annual training. Evidence: Person A has 2.5 of the required 16 hours of annual training for 2015. Plan of Correction: I have been doing online trainings and I will finish the 13.5 training hours in a month.
Standard #: 22VAC40-191-40-D-1-c Description: Based on a review of 4 household member's files, it was determined that two files did not have up to date repeat central registry findings on file every three years. Evidence: Person B and C's central registry findings are dated 8/2/13. Plan of Correction: I will be sending off for these background checks on Saturday.
