54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 11 children, totaling 27 points. The children were observed finishing lunch, assembling puzzles, singing songs, and preparing for their afternoon nap. The children were served cheese pizza, bananas, strawberries, sliced grapes, and milk for lunch today. This inspection was completed between approximately 11:00 am and 12:45 pm. Standard #: 22VAC40-111-700-A Description: Based on a review of children's medications, it was determined that one medication was on site without a completed authorization form. Evidence: One child's emergency prescription medication was on site without an authorization form completed and signed by the parent. Plan of Correction: The parent supplied detailed instructions for use of the medication. I will ask the parent to complete the authorization form.
Standard #: 22VAC40-111-710 Description: Based on a review of children's medications, it was determined that one prescription medication was on site without the required labeling. Evidence: One child's prescription medication was on site without the prescription label and the original medication packaging. Plan of Correction: The parent supplies the necessary amount to be given daily. The parent brings the medication in a medication pill organizer. I will ask the parent to supply the prescription label.
Standard #: 22VAC40-111-730-D Description: Based on observations, it was determined that one child's prescription medication was not locked. Evidence: One child's emergency prescription medication was stored on an upper level shelf beyond the children's reach, but the medication was not locked. Plan of Correction: The medication cannot be locked because it must be immediately accessible if needed. I will ask the parent to bring a doctor's stating the medication cannot be locked.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's medications, it was determined that two records were not complete. Evidence: 1. Child record #2 does not include: the provider's signature indicating that she viewed the child's original proof of identification. 2. Child record #3 does not include: the provider's signature indicating that she viewed the child's original proof of identification; a signed "Provisions of the Emergency Plan" document; a signed "Provider's Medication Policy" document. Plan of Correction: All files will be completed.
Standard #: 22VAC40-111-210-A Description: Based on a review of caregiver training records, it was determined that one caregiver did not attend at least 16 hours of training during the past year. Evidence: The current assistant caregiver did not complete any training courses during the past year. Plan of Correction: I have been enrolled in college courses for early childhood education for the past year. I have trained her on the information I've learned ongoing throughout the year. I will make sure she completes her own training this year.
Standard #: 22VAC40-111-280 Description: Based on observations, it was determined that one potentially hazardous substance was not stored in a manner to keep it inaccessible to the children. Evidence: In the backyard play area, an open bag of charcoal was stored on an open shelf beneath the grill. Plan of Correction: The charcoal will be moved.
Standard #: 22VAC40-191-40-D-1-c Description: Based on a review of background checks, it was determined that the background checks for two adults have not been renewed as required. Evidence: The following background checks have expired: The provider's Sworn Disclosure Statement (SDS) expired on 7/30/15; her Child Protective Services (CPS) registry check expired on 3/7/16; her Criminal Record Check (CRC) expired on 2/27/16. In addition, the same background checks for one adult family member living in the home expired on the same dates. Plan of Correction: We will both sign a new SDS form today. I will submit new background check requests for both of us this weekend.
