Comments:
An unannounced monitoring inspection was conducted today. Nine children were present with the provider and two assistants. Total 24 points with three caregivers. The children were playing inside with various toys, and the infants were being fed and having floor play time. Two children are new since the last inspection. Their records were reviewed today. The provider has MAT certification, but is not currently administering any medication. The violations cited at the last inspection have been corrected. Description: Based on record review, it was found that there was no documentation in two children's records, that all the required information was provided to their parents in writing.

Plan of Correction: This written information will be provided to the parents and their receipt of the information will be documented.
