Comments:
An unannounced monitoring inspection was initiated today at 11:50am. The physical plant, 3 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 9 children, totaling 18 points, under the supervision of 3 caregivers. Upon arrival to the FDH, there were 4 children with the Provider (6 pts) on a field trip to the swimming pool and 5 children at the FDH (12 pts) under the supervision of 2 caregivers. During the inspection, children were observed playing outside, cleaning up for and eating lunch, and resting at nap time. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served hot dogs (w/ a bun), broccoli, corn and milk. Standard #: 22VAC40-111-900-A
Description: Based on observations, there were foods served to children under four years that were not prepared in a way to prevent choking. Evidence 1. Tthere were 4 children in care (under the age of 4 years old) who were served hot dogs for lunch. These were given to the children whole to eat with their hands.

Plan of Correction: Once the caregivers were notified of this, they cut the hot dogs.

Standard #: 22VAC40-111-120-B
Description: Based on file review, 1 out of 3 caregiver records does not have all required information on file. Evidence 1. Caregiver #2's record is missing the following: start date of employment, verification of age, and the name address and telephone number of a person to be notified in an emergency.

Plan of Correction: Caregiver #1 will provide this information.

Standard #: 22VAC40-111-230-A
Description: Based on file review, 1 out of 3 caregiver records does not have written documentation of receiving orientation training. Evidence 1. Caregiver #2 does not have orientation training on file.

Plan of Correction: The caregiver will complete this.
