An unannounced monitoring inspection was initiated today at 11:15am. The physical plant, 2 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 4 children (plus the Provider's child), totaling 12 points, under the supervision of 2 caregivers. During the inspection, children were observed enjoying free play, working on learning the alphabet, listening to music and singing songs . The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children will be served macaroni and cheese, green beans, apple sauce and milk. The Provider is certified in MAT (medication administration training), CPR and First Aid. There are no children with any medication needs at this time. Description: Based on interview, the Provider does not have a menu for the current one-week period.

Description: Based on file review, 1 caregiver's record does not have all required information on file. Evidence 1. Caregiver #2 does not have documentation that two or more references as to character and reputation as well as competency were checked before employment began.

Plan of Correction: The Provider will compete this.

Standard #: 22VAC40-111-60-A
Description: Based on file review, 1 out of 4 children's records has not been updated annually. Evidence 1. Child #4's record was last updated on 2/23/14.

Plan of Correction: The Provider will have the parent review the child's record (make changes/updates if needed), then sign and date it.

Standard #: 22VAC40-111-90-C
Description: Based on file review, 1 out of 3 children's records does not have updated immunizations every six months up to the age of 2 years. Evidence 1. Child #2 is 1 year old. The most recent immunization record on file was completed on 12/30/15.

Plan of Correction: The Provider will ask the parents for this.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on file review, caregiver #2 does not have the findings of a criminal history record check (CRC) before 30 days of employment at the facility ends. Evidence 1. Caregiver #2 began working at the FDH in March 2015. There is no completed CRC on file.

Plan of Correction: The Provider will complete this.
