Technical Assistance:
Discussion held with provider about ensuring the menu has the date and not just the day of the week posted. Discussion held with provider about ensuring an acceptable substitute is called in when provider has to leave the home due to an emergency.

Comments:
An unannounced monitoring inspection was conducted today between 10:45 am and 12:10 pm. At the start of this unannounced monitoring inspection there were a total of 5 children (20 points) with 1 caregiver, which exceeded the maximum allowable points for safe supervision. A total of 4 children?s files and 3 staff/household member?s files were reviewed. The inspector observed children: eating, napping and playing in bouncy seats. Violations:
Standard #: 22VAC40-111-50-A
Description: Based on record review, the Family Day Home did not keep a written record of children in attendance each day. Evidence: Only four of the five children in care on the date of inspection were marked as present.

Plan of Correction: Provider will ensure children are marked in accurately.

Standard #: 22VAC40-111-60-B
Description: Based on record review, not all children's records were complete. Evidence: Child # 1, of 4 records reviewed, did not contain the child's health insurance policy number.

Plan of Correction: Provider will obtain information from the family.

Standard #: 22VAC40-111-320-A
Description: Based on observation, the family day home did not ensure all small appliances were unplugged when not in use. Evidence: A coffee pot was found to be plugged in on the counter in the kitchen.

Plan of Correction: Provider will unplug coffee pot.

Standard #: 22VAC40-111-320-B
Description: Based on observation of childcare area, not all electrical outlets had protective covers when not in use. Evidence: An electrical outlet to the left of the front door was discovered to be missing a protective covering in the top outlet.

Plan of Correction: Provider will inset a protective covering.

Standard #: 22VAC40-111-570-A
Description: Based on observation, the Family Day Home did not ensure that a caregiver did not exceed 16 points by using the prescribed point system. Evidence: 1. At the beginning of this inspection (10:45 am) 5 children were in care totaling 20 points, four over the maximum allowed) under the care of one substitute provider. 2. The provider returned to the Family Day Home at approximately 11:02 am meeting compliance for the maximum points allowed per caregiver.

Plan of Correction: We will not be over points again.
