Unannounced renewal inspections were conducted on 8/17/2016 from 9:50 - 11:05 and on 8/19/2016 from 10:40 - 11:20. There were six children = 7 points in care on both days. On the first day there was one caregiver and on the second day there were two caregivers. The children played outside with bubbles, large plastic balls and a bowling set. The exit interview was conducted with the licensee. The violation notice will be sent by U.S mail for completion.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the licensee did not maintain all furnishings inside the family day home in safe, operable condition. Evidence - One leg on the white wooden child size bed was observed to be cracking. There were two black shelf unites in use which were observed to not be stable. The back is coming off of one of the units.

Plan of Correction: The bed has been replaced And the self [sic] unites will be replace [sic]

Standard #: 22VAC40-111-320-I
Description: Based on observation, the licensee did not maintain one of the smoke detectors in working condition. Evidence - The smoke detector in the back play room was not working due to a dead battery.

Plan of Correction: New battery has been replaced

Standard #: 22VAC40-191-50-B-1
Description: Based on a review of staff records, the licensee did not update the Background checks every three years for one person. Evidence - The Background Checks for the licensee expired in March of 2016.

Plan of Correction: The Background Check will be update.
