Comments:
The following inspection took place between the hours of 9:00 am through 10:45 am. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 6 children, 1 being provider's child (21 points) with 2 caregivers. A total of 3 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, blocks and snack. Violations:
Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review. Evidence: There is no written documentation of an annual update to the emergency plan, last plan dated 2/27/15.

Plan of Correction: I will review the plan and sign.

Standard #: 22VAC40-111-830-B
Description: Based on record review, shelter-in-place procedures shall be practiced a minimum of twice per year. Evidence: There is no documentation that shelter-in-place drills have been practiced.

Plan of Correction: We will practice a drill this month.

Standard #: 22VAC40-111-50-A
Description: Based on record review, the family day home shall keep a written record of children in attendance each day. Evidence: There was no documentation of attendance for yesterday 9/7/16 or today.

Plan of Correction: I will write the attendance for yesterday and today, and maintain daily.
