Technical Assistance:
The provider shall incorporate a larger selection of multi-cultural toys/materials. In addition, the children's toys and materials shall be made more readily accessible to the children. All current children shall be encouraged to transition to more age-appropriate eating and drinking practices.

Comments:
An announced initial inspection for licensure was conducted today. The provider and one assistant caregiver were caring for 4 children, totaling 12 points. The children were observed playing with toys, interacting with each caregiver, and preparing for lunch. The children were to be served rice, string beans picked from the provider's garden, squash, lentils, strawberries, and milk for lunch today. This inspection was completed between approximately 10:15 am and 12:00 pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-770
Description: Based on a review of emergency supplies, it was determined that the provider does not have all required supplies. Evidence: The provider does not have a weather band radio.

Plan of Correction: I will buy the correct radio.

Standard #: 22VAC40-111-790
Description: Based on observations, it was determined that the provider has not posted all required emergency telephone numbers. Evidence: The provider has not posted the poison control phone number.

Plan of Correction: I will post the phone number today.

Standard #: 22VAC40-111-900-B
Description: Based on observations, it was determined that the children were permitted to lie down while drinking. Evidence: During this inspection, all four children in care were observed lying down on the floor while drinking from their bottles.

Plan of Correction: I will make sure they sit in their high chairs or at the table when eating and/or drinking.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that the records are not complete. Evidence: 1. All children's records are missing the following: documentation of each child's first date of attendance; documentation showing that the provider has viewed each child's original proof of identification; a signed "authorization to go off the premises" document; a signed "declaration of liability insurance" document; a signed "medication policy" document. 2. In addition, child record #2 and #3 do not include: the parent's medical insurance policy number; a physical exam report.

Plan of Correction: The parents will be asked for all missing information.

Standard #: 22VAC40-111-70-A
Description: Based on a review of children's records, it was determined that the provider has not accurately completed the "Information for Parents" document. Evidence: The provider has not included her termination policies on the "Information for Parents" document.

Plan of Correction: I will add the information and make parents aware of the policy.

Standard #: 22VAC40-111-690-D
Description: Based on interviews, it was determined that the provider does not clean the diaper changing mat as required. Evidence: The provider reported to the Inspector that she does not clean the diapering mat with soap and water prior to sanitizing it with the mixed bleach/water solution.

Plan of Correction: I will begin using soap/water in addition to the bleach/water that I've always used.
