22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted today, 9/7/16. The violations from the previous inspection have been corrected with one repeat violation found. A violation notice was completed during the inspection. During today's inspection the children played with the available toys, washed hands had lunch and then took a nap. Diapering and table sanitizing was also observed. A review of adult and children's records was conducted during today's inspection. Should you have any questions please do not hesitate to call. Inspection began: 10:25 am and ended approximately 1:20 pm.
Standard #: 22VAC40-111-210-A Description: Based on review of the staff records the provider failed to have a current first aid and CPR certificate. Evidence: 1. Staff #1 & 2 have a first aid and CPR certification that expired May 2016. Plan of Correction: The provider has called requesting a class and are waiting for the trainer to schedule this class. The provider will contact the trainer again to have this class scheduled.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review of household members records the provider failed to obtain background checks within the required time frames. Evidence: 1. Staff #3 has a criminal record check that expired 8/16/15 and a central registry check that expired 8/13/15. These background checks are to be repeated every three years. Plan of Correction: The provider has sent off these requests approximately 8/17/16. The provider will check on these if not returned within the week.
Standard #: 22VAC40-191-40-D-5-a Description: Based on review of records the provider failed to obtain a central registry check on two household members within 30 days of becoming 14 years old. Evidence: 1. Household members #1 & 2 turned 14 years of age on July 1, 2016 and there has not been a central registry check completed. Plan of Correction: The provider will have these forms filled out and will send them off.
