Discussed the following with the Provider: - Caregiver/household member #3 will need to complete a sworn disclosure statement prior to 9/19/16. - documentation of evacuation drills - policies regarding expired epi-pens
An unannounced renewal inspection was initiated today at 12:00pm. The physical plant, 4 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 8 children, totaling 21 points, under the supervision of 2 caregivers. During the inspection, children were observed resting at nap time. Earlier today, the children enjoyed recess outside at a playground nearby, practiced writing, danced to music and colored pictures. Today, the children were served chicken, rice, vegetables, grapes and milk. The Provider is currently certified in MAT (medication administration training), CPR and First Aid. Standard #: 22VAC40-111-100-A Description: Based on file review, 1 out of 4 children's records does not have the results of a physical examination on file. Evidence 1. Child #1 does not have a physical on file. Plan of Correction: The Provider will ask the parents for this.
Standard #: 22VAC40-111-60-B Description: Based on file review, 1 out of 4 children's records does not have all required information. Evidence 1. Child #1 does not have documentation of her start date. 2. Child #2 does not have a second required emergency contact in case the parents cannot be reached. Plan of Correction: The Provider will document the start date and ask the parents to provide a second contact.
Standard #: 22VAC40-111-180-A Description: Based on file review, 1 out of 4 caregiver/household members does not have a TB test/screening completed within the past two years. Evidence 1. Caregiver/household member #3's most recent TB test/screening was completed on 6/6/14. Plan of Correction: This will be completed again.
