Unannounced monitoring inspection was conducted beginning at 3:15 and ending at 4:45, When the LI arrived there were two children leaving and 1 child in care. Reviewed one family file and 4 children files - see violation notice. Discussed in detail regulation for always assuring that there is complete paperwork. Standard #: 22VAC40-111-50-A Description: Based on observation and interview with the provider, ut was determined that the daily attendance sheet was not accurate. Evidence: The attendance sheet for this week has two children listed for today. There were a total of four children in care today, but three of them have left. No one had been marked out. Plan of Correction: The attendance sheet will be filled out with each child's name as they arrive and the name will be crossed out when the child leaves.
Standard #: 22VAC40-111-60-A Description: Based on a review of children's files, it was determined that there was no file for 1 child and incomplete files for two children. Evidence: There was no file For child D. Child D was here today only as a favor to his mother; Child A who was enrolled on 3/28/16 did not have a physical and shot record on file. Shot records are to be on file before the child starts care and the physical within 30 days of enrollment. Child C who enrolled on 1/2/16 did not have a physical on file and the Liability Insurance Declaration is not filled out. Plan of Correction: The Provider will review all files and assure that they are all complete.
Standard #: 22VAC40-191-50-A-2-a Description: Based on a review of the family files, it was determined that one family member did not have a repeat Child Protective Services (CPS) form. This was previously cited at the inspection of 1/5/16. Evidence: The CPS form for Family member #1 was resubmitted but was not returned signed by the Central Registry. Plan of Correction: The Provider will call the Central Registry to find out what happened to the report.
