An announced Initial inspection was conducted today there are 3 children present (8 points) with 2 adults.
2 adult and 3 children's files were reviewed. See Violation notice form missing information.
I cannot submit your application for approval until all required background documents have been viewed. You will need to make arrangements to bring the original documents to the office for documentation. All children are within 7 day of the requirement for documenting Proof of Birth. 2 children within 30 for physicals to be on file.

Standard #: 22VAC40-111-770 Description: The radio available is not a weather band radio. Plan of Correction: I will purchase a new weather band radio.
Standard #: 22VAC40-111-800-A Description: There is no written emergency preparedness plan. (Template provided) Plan of Correction: I will complete the model form.
Standard #: 22VAC40-111-60-B Description: Based upon a review of files, children's files are incomplete. Evidence: 3 files were reviewed. Missing information includes: *Parent's work information - address information. (Children 1-3) * Complete emergency contact information for two contacts. (Children 1-3) Plan of Correction: I will switch to using the model DSS forms, and have the parents complete the information.
Standard #: 22VAC40-111-70-A Description: Based upon a review of files, children's files do not contain documentation of written acknowledgment of all required policies and procedures. Evidence: 3 of 3 files reviewed were missing documentation of policies and procedures. Missing information includes but is not limited to: Liability insurance declaration, Decision to Administer Medication, Provisions of the Emergency Preparedness Plan, the DSS web site address, parent's custodial rights statement, notification of a pet in the home, schedule of the day. Plan of Correction: I will ask the parents to complete the new (model) DSS forms.
Standard #: 22VAC40-111-460-D Description: Based upon a review of files, there is no documentation of current rabies vaccination for the dog. The groomer confirmed the vaccination is current; however, there is no certificate on file. Plan of Correction: I will contact the clinic to get the documentation of the shots.
Standard #: 22VAC40-191-40-C-1-a Description: Based upon a review of files, the provider's file is incomplete. Evidence: Adult A does not have a completed Criminal Record Check (CRC). Searches have been mailed, but not received. Plan of Correction: I will follow up with Richmond to see where the documents are.
Standard #: 22VAC40-191-40-D-1-a Description: Based upon a review of files, one adult file is incomplete. Evidence: Adult B's file does not contain documentation of a completed CPS search. Plan of Correction: I will contact Richmond to see where the searches are.
