Technical Assistance:

An unannounced renewal inspection was made on 9/20/16 at approximately 9:30 a. m. and concluded at 11:30 a. m. The inspector met with the provider and Family Day Home standards were reviewed. There were 12 children present with points equaling 34 with the provider and two assistants present. The children were observed having free play with age and stage appropriate toys and watching a video. One observation was conducted. Conversations were held with the provider and two children. No medications are being administered. The last evacuation drill was documented on 8/12/16. Two shelter-in-place drills conducted on 5/27/16 and 8/12/16. Six children records were reviewed. One assistant record reviewed.

Standard #: 22VAC40-111-830-C
Description: Based on review of the emergency drill log the provider did not log every monthly drill. Evidence: On 9/20/16, the emergency evacuation log did not have a monthly drill documented for July 2016.

Plan of Correction: Per provider, I will record evacuation drills.

Standard #: 22VAC40-111-920
Description: Based on review of the menu some of the required information was not available. Evidence: On 9/20/16, the menu that was posted was not dated.

Plan of Correction: I will date the menu today.

Standard #: 22VAC40-111-990-B
Description: Based on discussion with the provider there were required items missing in the vehicle when children were being transported. Evidence: On 9/20/16, the provider stated that there was no first kit on the vehicle being used for transportation. Currently the family day home is providing transportation for two children, however there was no written parent permission slips on file for transporting the children.

Plan of Correction: Per provider, will get parent permission slips filled out today for transportation of children and will get a first aid kit for the vehicle today.

Standard #: 22VAC40-111-60-B
Description: Based on review of children records the provider did not have all required information on file. Evidence: On 9/20/16, after review of six children records, child #1, #2 and #3 did not have a second emergency contact, address or phone number, there was no medical insurance information including the policy number on file for child #2, #3, and #4.

Plan of Correction: Per provider, I will get the parents to fill in blanks in kids records today, I will get medical insurance information and policy numbers for kids records today.
