Technical Assistance:
1. All sections of the Information for Parents Form should be completed consistently for each enrolled child.

Conducted a unannounced monitoring inspection 11:10am-12:05pm. Observed 11 children = 35 points with the Provider, Assistant #2, and Assistant #2. Ratios and supervision were in compliance. Children were eating lunch, washing hands, having story-time, singing an alphabet song, and having indoor free-play with toys. Caregivers were attentive to the children's needs, assisting them with tasks, and playing with them on the floor. The daycare areas of the home were found to be clean, safeguarded appropriately, and well supplied with age appropriate toys and equipment. Lunch served: pasta with a meat/tomato sauce, blueberries, and milk. No medications reported to be on-site. Required documents were posted. Record keeping violations found. Description: Based on records reviewed, it was determined that 1 of 5 children's records did not include documentation of a physical exam with the first 30-days of attendance. Evidence: Child #5 - The first day of attendance was 7/5/16, and to-date there was no physical exam on-file.

Description: Based on records reviewed, it was determined that 2 of 5 children's records did not include documentation of required information. Evidence: 1. Child #1 - No address and telephone number for 1 of 2 emergency contacts, no policy number for the medical insurance. 2. Child #5 - No documentation of allergies and health information, no policy number for the medical insurance.
