Technical Assistance:
Consultation was given regarding the availability of the most recent violation notice for the parents to view.

An unannounced monitoring inspection was conducted on September 27, 2016. At the time of entrance there was 1 provider and 7 children totaling 14 points. The sample size consisted of 3 children's records and 1 provider record. Children were observed sleeping, eating afternoon snack, playing with dolls and in the housekeeping area. The provider does not administer medications. The inspection began at approximately 2:20 p.m. and ended at 4:10 p.m.. The results of the inspection were reviewed with the provider during the exit interview. Areas of non-compliance are identified on the violation notice.

Standard #: 22VAC40-111-240-B
Description: Based on observation, the provider did not ensure areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition. Unsafe conditions include, but not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself; splintered, cracked, or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; and protruding nails, bolts, or other components that could entangle or could snag skin. Evidence: During the inspection on September 27, 2016, the licensing inspector the lower door frame on the outside shed and the wood is deteriorating, cracked and splintered wood.

Plan of Correction: The provider will have it renovated by November 30, 2016 and the door will be blocked until repaired.

Standard #: 22VAC40-111-450-A
Description: Based on observation, the provider did not ensure doors with clear glass panels that reach within 18 inches of the floor shall be clearly marked with decorative objects such as pictures, art work, or decals at the eye level of children in care. Evidence: During the inspection on September 27, 2016, the front glass door is not clearly marked.

Plan of Correction: The provider will put stickers up there today.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider did not ensure that children's hands are washed with liquid soap and warm running water before eating and after toileting or diapering. Evidence: During the inspection on March 3, 2015, the licensing inspector observed only 2 out of 7 children wash their hands prior to eating and child #1 did not wash their hands after tolieting.

Plan of Correction: The provider will ensure children wash their hands.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on a record review and provider interviews, the licensee did not ensure that a family day home provider had obtained updated background checks (sworn statement, search of central registry and criminal history record check) before three years since the dates the background checks were obtained. Evidence: During the inspection on September 27, 2016, the required background checks had not been updated as the most recent sworn statement was dated 7-23-2013 and the most recent search of central registry was dated 8-15-2013.

Plan of Correction: The provider will submit new background checks today.
