An unannounced monitoring visit was conducted during the Conditional licensure period. Three children were present with the caregiver for nine points. Children were observed in free play and clean up in preparation for lunch. Lunch to be served today was pizza, vegetables, apples and milk. Attendance and emergency evacuation drills were current. No medication was reported to be on-site today. The provider does have an assistant if needed when additional children are present. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-240-A Description: Based on observation, two areas of the home have not been maintained in a safe and operable manner. Evidence: 1. The door handle to the screen door is missing. The children exit out the door to use the back porch area as well as a secondary exit for emergency evacuations. 2. The bottom of the shed is peeling paint in multiple areas. Plan of Correction: Install door handle onto screen door. Replace bottom portion of shed wall and repaint new section.
