Standard #: 22VAC40-111-100-A Description: Based on a review of children's records, two children (#2 and 5) do not have documentation of a physical examination obtained within 30 days of their start date. Evidence: Child #2 has a start date of 7/15/14 and child #5 has a start date of 7/25/16. Both files are missing a physical. Plan of Correction: I will ask the parents to provide me with this documentation.
Standard #: 22VAC40-111-520-C Description: Based on direct observation and viewing manufacturer dates, 5 cribs do not meet current U.S. Consumer Product Safety Commission requirements. Evidence: Cribs manufactured prior to June 28, 2011 do not meet Consumer Product Safety Commission standards and have been recalled. The most recent date found on any of the 5 cribs present today is August, 2006. Only two cribs are required for the ages of children in care. Plan of Correction: We will replace the cribs that we need for children in care and dispose of any cribs that do not meet U.S. Consumer Product Safety Commission requirements.
