Comments:
An unannounced monitoring inspection was conducted by two inspectors today, 10/19/16. There were four children in care for a total of nine points with one caregiver. The children were observed playing with play dough and preparing for lunch and cleaning up the toys for lunch. Hand washing was also observed. Two adult and two children's records were reviewed. The violation from the previous inspection has been corrected. Areas of non-compliance are identified on the violation notice. Should you have any questions please do not hesitate to call. Inspection began: 9:30 am and ended 11:45 am.

Standard #: 22VAC40-111-90-A
Description: Based on review of the children's records the family day home failed to obtain an immunization record. Evidence: 1. The inspector reviewed two children's records. One of two children's records reviewed did not contain an immunization record. Documentation of immunizations must be provided before the child attends care.

Plan of Correction: The provider will have the parent bring the immunization record to the provider.
