- Technical Assistance:
Consultation was given on storage of supplies to ensure ample room in play area off of kitchen and not to use the changing table for that purpose, removing window screens from backyard area and gas can should be stored away from outside day care space. Children were not outside during this inspection.
An unannounced renewal study was conducted during the Conditional licensure period. The provider was closed the month of August. Five children were present with two caregivers for nineteen points which is within ratios for supervision. Children were observed resting, playing in room off of the kitchen area and hand washing. Reviewed basement play space as well as outside back yard area. Emergency drills and attendance was up to date. Lunch to be served today was fish sticks, grapes and milk. 3 cribs are present and were inspected at the initial study. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-120-B Description: Based on record review, an assistant record is incomplete. Evidence: Assistant #1 is missing an emergency contact and 2 references prior to hiring. Plan of Correction: An emergency contact and 2 references have been added to assistant #1's record.
Standard #: 22VAC40-111-60-B Description: Based on record review, 1 of 2 children's records reviewed was incomplete. Evidence: Child #1 was missing parent work address and health insurance policy number. Plan of Correction: Parent work address and health insurance policy number have been added to child #1's record.
Standard #: 22VAC40-111-200-A Description: Based on record review, orientation training has not been conducted for the two assistants. Evidence: Documentation of orientation training was not in either Assistant #1 or Assistant #2's personnel file. Plan of Correction: Documentation of orientation training has been added to assistant #1 and #2's personnel files.
Standard #: 22VAC40-111-530-F Description: Based on observation and provide statement, soft bedding was present in an occupied crib. Evidence: Child #2 was sleeping upon arrival upstairs. When the Inspector went upstairs to check on the child, the child was no longer in the crib. However, the provider stated that it was the crib the child had used. Three pillows were still in the crib and confirmed to be in the crib when the child was asleep. Plan of Correction: Pillows have been removed from the crib.
