- Technical Assistance:
Discussion was held on the topics of current enrollment and current staffing. Discussion was also held on the local zoning regulations (approved on 01/02/2014 by the County of Loudoun and are posted on the county web-site) and how they apply to the Family Day Home (FDH) license. A grace period was set (now expired) until 05/01/2014 by the County of Loudoun for any (at that time) State licensed FDH Provider to submit a Minor Special Exception application, if required, or Zoning Permit application to establish a Child Care Home use that conforms to all of the applicable Zoning Ordinance regulations as adopted with ZOAM-2012-0005 and diligently pursued the approval of such application. A notification letter dated 02/21/2014 and sent to all FDHs within the county. Therefore, the current capacity of the FDH shall be reduced in order to reflect the approved capacity for a town-home in Loudoun County. Until such time that the FDH submits a Minor Special Exception application and obtains approval for a higher number, the FDH capacity shall remain no higher than 9.
The following inspection took place between the hours of 11 am through 1 pm. Today, I met with the Provider on site at her Family Day Home located near Leesburg, Virginia. At the start of this unannounced renewal inspection there were a total of 3 children (8 points) with 1 caregiver. A total of 6 children?s files and 2 staff/household member?s files were reviewed. Standard #: 22VAC40-111-100-A Description: Based on review, 1 of 6 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (10/19/2016), documentation of a physical for Child #6 was not available for review. 2. The start date for Child #6 was listed as 09/06/2016. Plan of Correction: The Parent of Child #6 will be reminded and the documentation of a physical shall be obtained.
Standard #: 22VAC40-111-180-A Description: Based on review, 1 of 2 staff/household member's files reviewed indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening (TB) documentation (in accordance with the requirements in 22 VAC 40-111-170), every two years (from the date of last documented screening or more frequently as recommended by a physician or the local health department). Evidence - 1. On the date of inspection (10/19/2016), the most current TB documentation on file for Member #1 was dated - 09/29/2014. 2. This review was confirmed on the date of inspection by the Provider. Plan of Correction: Updated documentation shall be obtained for Member #1 and placed on file.
Standard #: 22VAC40-111-240-A Description: Based on observation and interview, a furnishing of the Family Day Home (FDH), inside, does not have supporting documentation that states that it meets the safety standards set by the Federal Government, Consumer Product Safety Commission (CPSC) that went into effect on December 28, 2012. Evidence - 1. The date of manufacture on the FDH crib was listed as from 2002. 2. On the date of inspection (10/19/2016), the manufacture was contacted and confirmed that this crib does not meet the new federal regulations. 3. The Provider stated (during an interview conducted on the date of inspection) that the crib was recently given to her by a parent. Plan of Correction: The crib shall no longer be in use and shall be replaced by an approved crib that meets the CPSC regulations.
