Discussed the point value with the provider. Reminded provider to renew CPR by end of December. Provided regular field trrip form for the walking trips and the diaper cream form.
The following inspection took place between the hours of 9:00 am through 10:30 am. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 6 children (17 points) with the provider. A total of 3 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, books, bathroom breaks, and hand washing. Standard #: 22VAC40-111-60-B Description: Based on record review, a child's record was not complete. Child #1 and #2's record did not contain 2 emergency contact information and the policy number for the medical insurance. Child #3 record did not contain the policy number for the medical insurance. Plan of Correction: I will have the parents give me the information.
Standard #: 22VAC40-111-570-A Description: Based on observation, the provider exceeded the 16 points. Evidence: There were 6 children in attendance upon my arrival at 9:00 am, 17 points with just the provider. Plan of Correction: My assistant is at school this morning. I have a child leaving in January and 2 that come part time. I will arrange the children's schedule so that I am within the 16 points when by myself.
