Comments:
An unannounced monitoring inspection was completed on October 26, 2016 from 9:50 to11:55am . The purpose of this visit was to measure the provider?s compliance with licensing standards. There were 7 children in care, with a total of 18 points with 2 staff persons. The children ranged in age from infant to 4yrs . The children were observed engaging in free play and singing songs. The older children reviewed the letters of the alphabet. Lunch consisted of navy beans, broccoli, pineapples, biscuits and milk . There was no children?s medication stored on site. A sample of staff and children's records were reviewed. The last evacuation drill was conducted 9/21/16. The last shelter in place drill was conducted 3/22/16.

Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observations of the child care space, the provider failed to ensure that all prescriptions were locked. EVIDENCE: Observed the caregiver's prescription medication on the table accessible to children in care.

Plan of Correction: Corrected on site. Will keep in the lock box.

Standard #: 22VAC40-111-120-B
Description: Based on a review of staff records, the facility did not ensure that the record of staff persons contained all of the required documentation. EVIDENCE : The record of staff person #2 did not contain 2 references or verification of orientation training, and this staff began employment in July 2016.

Plan of Correction: Will contact two references for the staff person and will put training paper in the record.

Standard #: 22VAC40-111-60-B
Description: Based on a review of a sample of children's records, the provider did not ensure that all required documentation was in the children's records. EVIDENCE: The record of child # 5 did not contain parent acknowledgements of the medication policy and the provisions of the Emergency preparedness plan or the employment address for the parents. The parent information sheet in the record of child#6, was not signed by the parent.

Plan of Correction: Will get the parents to sign the papers this evening.

Standard #: 22VAC40-111-90-A
Description: Based on a review of a sample of children's records, the provider did not ensure that immunization records were obtained prior to a child's attendance at the facility. EVIDENCE: The record of child #5 did not contain a record of immunizations, and the child began attending the program on 10/3/16.

Plan of Correction: Will ask the mother this evening to get a copy of the immunization record.
