63.2 Liability Insurance Disclosure
Today an unannounced monitoring visit was conducted with the Provider. This visit took place from 10:30-12noon there were 4 children present with the Provider for a total of 10 points within the supervision guidelines. The children were observed having indoor free play, riding bikes, taking a morning nap, lunch, blocks and in transition for nap time. Lunch is provided by the parents. A complete inspection of the physical plant, children and provider records, fire drill log, attendance record, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-B Description: Based on a review of the children records, one file was incomplete. Evidence: Child #4's record did not include the father's complete work address. Plan of Correction: Provider stated that she will ask the parent for the work address today.
Standard #: 22VAC40-111-530-F Description: Based on observation of an infant during nap time, a infant was observed with a blanket wrapped around them. Evidence: Upon arrival, an infant was observed in a crib wrapped with a blanket around them. Plan of Correction: Provider stated that the child has a running nose and wanted to keep them warm. Provider removed the blanket during this inspection.
