Comments:
An unannounced renewal inspection was conducted today from 9:50 - 11:15. There were 8 children = 25 points with three caregivers. The children played with blocks, had circle time, sang songs, listened to a story and ate snack. The exit interview was conducted with the licensee. The violation notice will be e mailed for completion.

Violations:
Standard #: 22VAC40-111-120-B
Description: Based on a review of records, the licensee did not obtain an updated Sworn Statement every three years. Evidence - The records for the licensee and the licensee"s spouse contained Sworn Statements that expired in April of 2016.

Plan of Correction: The Sworn Statements were filled out & signed by my spouse & myself.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, the licensee did not obtain all of the required information for two children. Evidence - The record for child #2 did not contain the parent's signed permission to obtain emergency medical treatment for the child. The record for child #5 did not contain a street address/house number for the child's first emergency contact.

Plan of Correction: Child #2 : Got the parents signed permission to obtain emergency medical treatment for the child. Child #5 : Obtained the address of the emergency contact.

Standard #: 22VAC40-111-70-A
Description: Based on a review of children's records, the licensee did not give complete written information to two parents. Evidence - The record for child # 2 contained a Liability Insurance form with no child's name on it and there was no name listed on the Medication Policy form for who would give any necessary medication. The record for child #5 contained a Medication Policy Form that did not list the name of the person who would give any required information.

Plan of Correction: All this was fixed while inspector was still here at my house.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the licensee did not cover one electrical outlet in the facility. Evidence - There was one outlet observed in the sleep room that was not covered with a child resistant protective cover.

Plan of Correction: Was corrected while the inspector was still at my house.
