Comments:
An unannounced inspection was conducted on August 24, 2016 and September 20, 2016, in response to a self reported incident reported to the Valley Licensing Office on August 16, 2016. The initial areas of concern included the facility space, indoor temperature and supervision of children. An additional concern included transportation of children. Interviews were conducted, observations made and available documentation was reviewed. The preponderance of the evidence did not support the allegations and the complaint was, therefore, not valid. Although not referenced in the originally reported concerns, licensing standards 22 VAC 40-111-(2)-50-A, 22 VAC 40-111-(9)-690-C and 22 VAC 40-111-(9)-690-E were found to be in violation. Refer to the violation notice for additional information. Issues discussed included: documentation of daily attendance; diapering procedures; use of the garage; and permission for transporting children. A copy of the general written permission form for taking children off of the facility premises was provided. Please be reminded that it is the responsibility of the licensee to ensure ongoing compliance with the licensing standards. Standard #: 22VAC40-111-50-A
Description: Based on review of available attendance records and discussion with caregiver A on August 24, 2016 and September 20, 2016, the program failed to keep a written record of children in attendance on August 16, 2016. Evidence: 1. During review of available records on both August 24, 2016 and September 20, 2016, caregiver A was not able to locate the attendance record for children in care on August 16, 2016. 2. Caregiver A stated that she was not able to find the attendance record for August 24, .2016.

Plan of Correction: A written record of children in attendance will be completed each day children are in care. The attendance records will be kept in a folder.

Standard #: 22VAC40-111-690-C
Description: Based on observation of diapering/toilet training on September 20, 2016 and discussion with caregiver A, one child was not cleaned with a moist disposable wipe during a diaper change. Evidence: 1. On this date, the licensing inspector observed the diapering process. A child's diaper was removed, disposed of, and the child sat on the toilet. Then, a clean diaper was placed on the child. The child was not cleaned, as required. 2. Caregiver A confirmed that the child was not cleaned prior to receiving a clean diaper.

Plan of Correction: In the future, children will be cleaned with a moist disposable wipe during each diaper change or after toileting accidents.

Standard #: 22VAC40-111-690-E
Description: During a diaper change on September 20, 2016, program staff failed to dispose of a soiled diaper in a leak-proof or plastic lined storage system that was foot operated or "hands free." Evidence: 1. On this date, the L.I. observed caregiver A dispose of a soiled diaper in an open trash receptacle accessible to the children in care. 2. Caregiver A confirmed that a foot operated or "hands free" covered storage system was not used for the disposal of the diaper.

Plan of Correction: In the future, A foot operated or "hands free" covered storage system will be used for the disposal of soiled diapers and wipes.
