54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring inspection was conducted from 12:34pm to 1:15pm. At the time of inspector's arrival there were 6 children, ages 11mos-4yrs, for a total of 14 points, in care with the provider. The records sample size consisted of 6 children's records and 3 provider / assistant/ household member records. Children were observed engaging in creative play, nap and learning activities. The provider stated that no medication is administered to children while in her care. Injury reports were also reviewed during this inspection. Areas of noncompliance are detailed in the violation notice and were discussed with the provider during the exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-111-90-C
Description: Based on record review it was determined that the provider did not obtain documentation of additional immunizations for children once every six months for children under the age of two years. Evidence: The record for child #5(DOB 1/8/15) did not contain updated immunization records. The last record was dated 1/11/16.

Plan of Correction: I have her updated papers.

Standard #: 22VAC40-111-280
Description: Based on observation it was determined that the provider did not ensure that all poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children. Evidence: There was a container of Clorox wipes in the unlocked cabinet under the sink in the bathroom the children in care use. The wipes carry a "keep out of the reach of children" label.

Plan of Correction: Provider removed the wipes during this inspection. They are in a locked cabinet.

Standard #: 22VAC40-111-350-E
Description: Based on observation, it was determined that the hot water temperature was not maintained between 105-120 degrees Fahrenheit by the provider. Evidence: The hot water temperature was measure by inspector at 125.9 degrees Fahrenheit at the time of the inspection. Steam was observed from the tap. This is the same water supply that the children access to wash their hands.

Plan of Correction: I will keep a close check on the hot water.
