An unannounced Monitoring inspection was conducted today. There are 5 children present (11 Points) with one adult. Children were resting quietly during the inspection. 5 children's file were reviewed See violation notice form missing information. Emergency escape drills, rest mats, menu are all in compliance. Standard #: 22VAC40-111-60-B Description: Based upon a review of files, children's file are incomplete. Evidence: 2 of 5 file reviewed were missing complete documentation of emergency contact information. (Children 1, 2) Plan of Correction: I have contact the parents to obtain the information.
Standard #: 22VAC40-111-70-A Description: Based upon a review of children's files, parents have not been provided in writing all required policies and procedures. Evidence: 5 of 5 files reviewed were missing documentation of policies. Missing information includes but is not limited to: Custodial rights statement, Liability insurance declaration, Decision to administer medications, Provisions of the emergency preparedness plan. (Children 1,2,3,4,5) Plan of Correction: I will print out the model DSS forms and have the parents sign them.
