54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Discussed when MAT training is required with provider. No topical skin products found on site but discussed when they are, the need to label each topical item with the child's name. Provider stated that new wallpaper will be installed in kitchen located in basement.

Comments:
An unannounced Renewal inspection was conducted this date. Walk thru of entire household was conducted, three adult household members were present along with provider. Two children present today with provider, for a total of 4 points. Observation of drop off and morning playtime. Inspection began at 10:30am and ended at 12:30pm, to include the exit interview. All violations were reviewed with provider and an acceptable corrective action was provided. Standard #: 22VAC40-111-830-D
Description: Based on review of provider's monthly emergency response drills there no documentation of shelter in place drills. Evidence: No shelter in place drills to date were found documented.

Plan of Correction: Will document shelter in place drills beginning this month.

Standard #: 22VAC40-111-920
Description: Based on observation, menu posted was not currently dated. Evidence: The menu posted on side of cabinet in lower level did not have today's date on the menu.

Plan of Correction: Provider will correct menu to include current date.

Standard #: 22VAC40-111-90-C
Description: Based on review of children's records, one child did not have documentation of current immunizations within the last six months. Evidence: Child #1(DOB: 9/13/2014) is under the age of 2 and last immunizations were dated 2015.

Plan of Correction: Provider will obtain most recent immunization record from child #1.

Standard #: 22VAC40-111-180-A
Description: Based on review of household member documentation, two of adult records did not have subsequent TB screenings in the past two years. Evidence: Household member #1 and #2's most recent TB screening were dated 9/2014

Plan of Correction: Provider stated both household members will obtain TB screenings this month.

Standard #: 22VAC40-111-240-A
Description: Based on observation of physical plant, areas were found not maintained in good repair. Evidence: Two wooden boards found on backyard fence were splitting and cracked with chipped paint, near the bottom of one board and the top of another one. Outside stairway had exposed areas of missing aluminum siding with nails accessible to children. One smoke detector was found to have an expired battery and was beeping during the inspection.

Plan of Correction: Provider's husband stated he will repair all areas.
