22VAC40-191 Background Checks for Child Welfare Agencies
Today an unannounced renewal inspection was conducted beginning at 11:45am. There were 2 children (8 points) present with the Provider. Two adult records and two children's records were reviewed. There are currently no medications on-site for children enrolled. Children were observed eating lunch and sitting at in interactive saucer table. There is a pack and play on-site and the Provider states that it is only used for quiet play time and not for sleeping. No children were observed sleeping in it during the inspection. As of 11/14/16 a renewal application has not been received in the licensing office for the license expiring on 12/24/16. Please be sure that it is complete and submitted to the licensing office prior to the license expiring. This inspection, including the exit interview, ended at 12:45pm. Standard #: 22VAC40-111-100-A Description: Based on a review of records, one child (#1) is missing documentation that a physical examination was obtained within 30 days of their start date. Evidence: Child #1 has a start date of 6/13/16. There is no documentation of a physical in their record. Plan of Correction: I will ask the parents to provide me with a physical for this child. I will will make sure I obtain a physical examination for children within 30 days of their start date.
Standard #: 22VAC40-111-210-A Description: Based on a review of annual training documentation, one caregiver (#1) did not obtain 16 hours of annual training. Evidence: In the current program year beginning December, 2015, caregiver #1 obtained 8 of 16 annual hours of training. Plan of Correction: I will attend and document all required training hours.
