Discussion was held on the topics of current enrollment and current staffing.
The following inspection took place between the hours of 1:30 pm through 4 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 8 children (22 points) with 2 caregivers. A total of 8 children?s files and 6 staff/household member?s files were reviewed. The children were engaged in the following: nap time, diaper changing, bathroom break, hand washing, snack time, music, toy play, games, song, and other organized activities. Standard #: 22VAC40-111-800-B Description: Based on observation, the most recent documented review of the Family Day Home's (FDH) Emergency Preparedness and Response Plan (EP&RP) was greater than one year. Evidence - 1. On the date of inspection (11/15/2016) the most recent documented review of the FDH's EP&RP was dated - 09/15/2015. 2. This plan was posted and observed on the parent's bulletin board on the steps down to the child care area of the FDH. Plan of Correction: The EP&RP shall be reviewed, if needed updated, and documentation of this review shall be done in writing.
Standard #: 22VAC40-111-100-A Description: Based on review, 1 of 8 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (11/15/2016), documentation of a physical examination was not available for review for Child #4. 2. Based on the information on file, the start date for Child #4 was listed as 08/08/2016. Plan of Correction: A physical examination for Child #4 shall be obtained and placed on file.
Standard #: 22VAC40-111-60-B Description: Based on review, 2 of 8 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (11/15/2016), the medical insurance information for Child #4 was not available for review. 2. The proof of identification documentation for Child #4 was not available for review. The start date for Child #4 was listed as 08/08/2016. Plan of Correction: All missing information shall be obtained and placed in the applicable child's file.
Standard #: 22VAC40-111-90-A Description: Based on review, 1 of 8 children's files reviewed indicated that the Provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before the child's first day of attendance in the Family Day Home (FDH). Evidence - 1. On the date of inspection (11/15/2016), an immunization page for Child #5 was not available for review. 2. The start date for Child #5 was listed as 11/07/2016. Child #5 was in attendance on the date of inspection. Plan of Correction: An immunization page for Child #5 shall be obtained and placed on file.
Standard #: 22VAC40-111-180-A Description: Based on review, 1 of 6 staff/household members' files reviewed indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening (TB) form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the last TB documentation. Evidence - 1. On the date of inspection (11/15/2016), TB documentation was not available for review for Member #3. 2. The most current TB documentation on file for Member #3 was dated - 10/22/2014. Plan of Correction: Current TB documentation for Member #3 shall be obtained and placed on file.
