Comments:
LI Anderson conducted an unannounced renewal inspection on Friday, November 18, 2016. Upon arrival at 2:15 PM there were five (5) children ages 15 months ? 4 years old and the provider present. Two (2) additional school age children arrived during the inspection. Children present totaled 13 points under the direct supervision of one (1) caregiver. Children & Staff/Household records were reviewed. Provider continues to participate in the USDA Child Care Food Program, does not provide transportation and does not administer medication. Violations found during the previous inspection were reviewed & have been corrected. Children were observed during departure procedures, afternoon rest, individual/structured tasks and self-initiated activities. An exit interview including review of the Risk Assessment & areas of non compliance found during today?s inspection were reviewed with the Provider on 11/18/2016. Inspection concluded at 4:45 PM.

Violations:
Standard #: 22VAC40-111-920
Description: Based on inspection, the Licensee failed to ensure the menu for the current one-week period is dated. EVIDENCE: The posted menu for the current week did not indicate the current dates.

Plan of Correction: Provider will date weekly menus

Standard #: 22VAC40-111-60-B
Description: Based on record review, the Licensee failed to ensure each child's record includes all required documentation. EVIDENCE: LI Anderson reviewed five (5) children records. Record for Child #4 did not include addresses of two designated persons to contact in case of an emergency and the parent cannot be reached. Record for Child #4 did not include the policy number of the child's medical insurance. Records for Child #2 and Child #4 did not include documentation of the annual updates review of the child's emergency contact information as required by 22 VAC 40-111-780 B.

Plan of Correction: Provider will have the parents to complete

Standard #: 63.2-1809.1-B
Description: Based on record review, the Licensee failed to have each parent/guardian acknowledge in writing that they have received written notice of liability insurance. EVIDENCE: Record for Child #1 did not include documentation from each parent/guardian acknowledging written receipt of liability insurance.

Plan of Correction: Provider will have parents sign liability insurance acknowledgement.
