National fingerprinting background checks go into effect on 9/30/17.
An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 8 children, totaling 22 points. The children were observed playing with toys, reading books, and interacting with each caregiver. The children were to be served rice with chicken and vegetables, apples, and milk for lunch later today. This inspection was completed between approximately 9:30 am and 10:40 am. Standard #: 22VAC40-111-900-B Description: Based on observations, it was determined that several children were permitted to walk around while eating. Evidence: Upon arrival, the Inspector observed four young children walking around while eating candy. Later in the inspection, one of these same children was observed walking around again while eating a cracker. Plan of Correction: The children had just finished celebrating a birthday when the Inspector arrived. I will make sure all children sit at the table or in a high chair when eating or drinking.
Standard #: 22VAC40-111-920 Description: Based on observations, it was determined that the provider did not post a menu as required. Evidence: The provider had not posted the menu for the current one-week period. Plan of Correction: I will post the menu each week.
Standard #: 22VAC40-111-120-B Description: Based on a review of one assistant caregiver record, it was determined that this record was not complete. Evidence: The file for the current assistant caregiver does not include: documentation of her first day of hire; emergency contact information; First Day Orientation Training; background check findings for Child Protective Services (CPS) and Criminal History Record Check (CRC.) Plan of Correction: I could not submit her background checks until she renewed her personal identification information. She just completed that. I will submit the CPS and CRC background check requests tomorrow. All other items will be completed and the documentation will be placed in her file.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #3 does not include: a signed "Information for Parents" document; documentation that the parents completed an annual review of the child's emergency contact information. 2. Child record #4 does not include a signed "Medication Policy" document. Plan of Correction: I will ask the parents for the missing information.
Standard #: 22VAC40-111-340-C Description: Based on observations, it was determined that the children's bathroom was not equipped as required. Evidence: Upon arrival, there were no paper towels and no liquid soap in the children's bathroom. Plan of Correction: The soap and paper towels were put in the bathroom immediately during the inspection. They had been moved for the children to wash their hands at the sink closer to the area where we had just celebrated a birthday.
