Comments:
The licensing inspector and licensing administrator conducted an unannounced monitoring inspection on November 18, 2016 from 2:00 pm to 5:20 pm. During this inspection, there were 10 children totaling 15 points being cared for by the provider. Upon the inspector's arrival, the children were napping in the designated area. The children's indoor play area and age appropriate materials to include toys, books, and various materials were observed for inspection. There were a variety of artwork and fall festive decorations displayed throughout the children's room. The inspector observed the required postings to include breakfast and lunch menus, fire and shelter in place drills displayed on the wall near the main entrance. The first aid kit and emergency supplies, posted telephone numbers, emergency preparedness and response plan were inspected. During this inspection, four children's and one provider record were reviewed for compliance of standards. The inspector completed observation of the children after the completion of their nap time, routine restroom break, playing outside at playground, interacting with peers and provider, snack time, and departing for the evening. The inspector also completed observation of napping area. The violation from the previous inspection on 05/18/16 was checked for correction. Standard #: 22VAC40-111-100-A
Description: Based on the review of children's records, the provider also did not obtain a physical examination for two of four children's records within one month after attendance.
Evidence: In child #1's record, first date of attendance on 9/29/14, it was observed that there was a missing physical record.
In child #4's record, first date of attendance on 11/16/15, it was observed that there was a missing physical record.

Plan of Correction: Per provider: I will contact children's parents and request documents in a week.

Standard #: 22VAC40-111-90-A
Description: Based on the review of children's records, the provider did not obtain documentation of immunization for two of four children's records which shall be obtained before a child may attend.
Evidence: In child #1's record, first date of attendance 9/29/14, it was observed that there was a missing immunization record.
In child #3's record, first date of attendance on 10/17/16, it was observed that there was a missing immunization record.

Plan of Correction: Per provider: I will contact children's parents and request documents in a week.

Standard #: 22VAC40-111-480-E
Description: Based on observation of the playground area, the center did not ensure all equipment was maintained in a safe condition. Evidence: The wooden fort with a slide attached was observed to have a protruding nail and splintered or cracked wood on the side.
Evidence: The wooden rail on the side of the wooden house had a protruding nail and the handrail was loose.

Plan of Correction: This was corrected during the inspection.
