Comments:
6 children present with 2 caregivers, a total of 15 points at this renewal inspection. The children were playing outside at the beginning of the visit , lunch was observed, the children had chicken stew and fruit salad for lunch. After lunch the children played and danced, lots of samba going on. The provider has lots of toys and interest areas set up. This visit was started at 10:45 and ended at 12:00. Standard #: 22VAC40-111-700-B
Description: The provider has one medication without the proper medication authorization. Evidence: Based on the review of medications and medication authorization the provider has one medication. The parent gave the provider the authorization for medication on a model form that is for bug spray and sunscreen. The form does not include information needed for over the counter medications.

Plan of Correction: I will return the medication to the parents and ask for the correct authorization

Standard #: 22VAC40-111-60-B
Description: The provider failed to get addresses for emergency contacts for one child. Evidence: Based on the review of 4 children's records , child C is missing the emergency contact addresses.

Plan of Correction: I will ask the parent for the information

Standard #: 22VAC40-111-80-C
Description: The provider failed to record the birth certificate number in the child's file. Evidence: Based on interview with the provider, child C is missing the birth certificate in the file because the parent asked for it to apply for a passport. The provider did not record the information on the child information form.

Plan of Correction: I will get it back from the parent.

Standard #: 22VAC40-111-690-D
Description: The diaper changing pad is torn and can not be sanitized without the solution soaking in to the foam.

Plan of Correction: I will buy a new one.
