An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 12:20 PM and departed at 2:30 PM. Eleven children, ages 2 years - 5 years, totaling twenty-two points, were in care with two caregivers. In the absence of the provider, the children were in the care of a substitute provider. Activities and transition from lunch time to quiet/nap time was observed. It was reported that no medication is administered to children in care and no children's injuries were reported. A sample size of four caregiver/household records and five children's records were reviewed. An exit meeting was conducted with the provider via telephone during which the plan of correction was obtained prior to closure of the inspection. Description: Based upon review of four caregiver/household records, the provider has not ensure that repeat background checks are obtained before three years since the dates of the most recent child protective services (CPS) central registry finding and most recent criminal history record check report. Evidence: 1. The most recent CPS central registry findings in the records provided for caregiver 1 and household member 2 are both dated 9/5/2013. 2. The most recent criminal history record check in the record provided for household member 2 is dated 08/23/2013. 3. There was documentation provided that the repeat CPS searches and the criminal history record were not requested until 10/26/2016.

Plan of Correction: The provider responded that in the future repeat background checks will be requested well in advance of the three year expiration dates.
