54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced morning inspection was conducted this morning. There were 4 children present with the Provider for a total of 8 points. The Provider has attended 10 hours in training for Subsidy program and has completed Recognizing Child Abuse. The children were involved in activities and the home was observed with no hazards seen. The Provider has a variety of age/stage appropriate materials available and accessible to the children. Files, posted documents, and license were all available for review. Standard #: 22VAC40-111-60-B Description: Record review of 2 children's files revealed the files are missing the following information: addresses for Emergency Contacts for child #1 and start date for child #2. Plan of Correction: I will record start date and request addresses.
