54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal study was conducted from 10:20 am - 11:30 am. There were six children in care with the provider and one assistant ages 12 months, 12 months, 19 months, 20 months, 2 years, and 2 years for a total of 18 points. The children were observed participating in free play and circle time activities. Lunch served today are diced hot dogs, applesauce, green beans, dinner roll, and milk. Six children's records were reviewed and in compliance. The provider and one assistant records were reviewed. The most recent emergency escape drill was conducted 11/2/16. Standard #: 22VAC40-111-240-A Description: Based on observation of the family day home it was determined that not all areas of the the home were safe condition. Evidence: The electircal outlet face cover below the parent board is cracked with the corner piece missing exposing sharp edges. Plan of Correction: The provider will replace the cracked outlet cover with a new one.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review of the provider record it was determined that not all background checks were repeated every three years. Evidence: The most recent Central Registry Check on file for the provider was dated 11/18/2013. Plan of Correction: The provider will complete a central registry background check and submit form for results.
