Thank you for your time and assistance with the unannounced monitoring inspection conducted today, December 8, 2016. The on-site inspection was held from 11:15 a.m. to 2:30 p.m. On this date, there were four children in the care of the provider (12 points). There were a total of four children enrolled in the program. There were four children's records reviewed, along with the records for the provider and one household member. The provider and two children were interviewed. There were no medications being administered at the time of the visit, as is the policy of the provider. The exception to compliance was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. Lunch was observed to meet USDA guidelines and consisted of cheese pizza, grapes, pears, and milk. All interactions between the children and the providers were age appropriate and positive. The Letter of the Week, I,i and this week they were working on Christmas activities. Some items reviewed during the visit included: emergency supplies, first aid kit, required posted information, menu, documentation of evacuation and shelter-in-place drills. Lunch, choice time, hand washing and bathroom procedures, infant feeding and care, diaper changing procedures, story time, and nap were observed during the visit. Standard #: 22VAC40-111-180-A Description: Based on record review and verification from the provider, she failed to obtain an updated tuberculosis (TB) screening within two years from the date of the last screening. Evidence: 1. The date of the provider's last TB screening was 10/28/2014 and she did not obtain an updated one until 12/07/2016. 2. The provider verified that she was late in getting her TB screening. Plan of Correction: In the future, I will pay closer attention to the date the screening needs to be done.
