Today's visit was a renewal inspection. The inspection began at 8:50am and concluded at 12:35pm. There were two providers present with four children in care and a total of seven points. During the inspection arrival of a child was observed, group activities were observed, lunch time was observed, four out of eight children's records were reviewed, the licensee's record was reviewed along with spouses/assistant record, a fourteen year old family member's CPS results were reviewed, and the record for an additional assistant, the emergency preparedness plan was reviewed, emergency supplies were reviewed, and the outdoor play area was reviewed. Discussion took place about moving the the licensing board to the entrance of the family day home. Your renewal application has been received in the licensing office and a recommendation regarding your license will be made upon the completion of today's inspection. Standard #: 22VAC40-111-920
Description: Based on the inspector's observations, the provider failed to have the current menu posted. Evidence: The inspector observed the menu for the previous week posted. The current week's menu is required to be posted.

Plan of Correction: The provider removed the menu to take a look at it upstairs. The menu was posted during the day of the inspection.

Standard #: 22VAC40-111-100-A
Description: Based on the inspector's review of four out of eight children's records, the provider failed to ensure the record for child included a physical exam within thirty days after the first day of attendance. Evidence: The record for child #4 has a documented start date of 6/29/16 and does not have documentation of a physical exam.

Plan of Correction: The provider will request that the parent provide a copy of the child's physical within one week of today's inspection.

Standard #: 22VAC40-111-70-A
Description: Based on the inspector's review of four out of eight children's records, the provider failed to ensure a child's record included all of the required policies/procedures. Evidence: The record for child #1 does not have documentation that the parent received the the family day homes regarding medication administration.

Plan of Correction: The provider will have the parent sign the medication policy upon pick up today.
