Comments:
An unannounced monitoring inspection conducted on December 6th 2016 between 11:00am and 12:45pm. There were 5 children in the direct care of 4 staff members. Children were observed eating lunch, building with blocks, being diapering, and napping. The day care space is clean, organized, and offers plenty of space for the children to play. A sampling of children's records, the staffing records, the evacuation drills, shelter in place drills, the physical space, and supervision were observed and inspected. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on observation, emergency evacuation procedures were not practiced monthly. Evidence: The last documented evacuation drill was conducted September 2016. In October a, shelter in place drill was conducted, but it does not take the place of a monthly evacuation drill.

Plan of Correction: I will conduct them at the end of each month.

Standard #: 22VAC40-111-900-B
Description: Based on observation, children were observed to be drinking while lying down. Evidence: 3 children, before nap time, were given bottles to drink while they were lying down on the ground or on a pillow.

Plan of Correction: It won't happen again.

Standard #: 22VAC40-111-180-A
Description: Based on record review, 1 of 4 records reviewed did not have documentation of a Tuberculosis Screening form. Evidence: Assistant 1 does not have documentation of a Tuberculosis screening.

Plan of Correction: I have spoken with the staff to get it completed or she will need to be terminated.

Standard #: 22VAC40-111-200-A
Description: A review of staff records today found that there was no documentation of orientation as required for assistant staff. Evidence: There was no form to demonstrate orientation for Assistant 1, 2 and 3.

Plan of Correction: I will do it right away once I have the form.

Standard #: 22VAC40-111-680-A
Description: Based on observation, an assistant did not wash their hands after diapering a child. Evidence: An assistant was observed finishing diapering a child, walking to the kitchen area, retrieving a bottle, and feeding the child.

Plan of Correction: I will talk with the staff today at the staff meeting at the end of the day.

Standard #: 22VAC40-111-690-D
Description: Based on observation the diapering surface was not nonabsorbent. Evidence: The diaper pad used for changing children has small tears in it leaving the foam inside exposed.

Plan of Correction: This was corrected during the time of inspection.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on record review, 1 of 4 records reviewed did not have a central registry check before 30 days of employment at the family day home ended. Evidence: Assistant #1 does not have completed documentation of a central registry search. The form on file is filled out, but not signed by the agency it was sent to.

Plan of Correction: i will send it back to the agency today.
