Comments:
An unannounced monitoring inspection was conducted with provider and assistant. There were nine day care children and two neighbor children present for 25 points. The children were waking from rest, arriving from school, and having afternoon snack. Snack served today: graham crackers and milk. There are no children receiving medication at day care today - provider is certified in Medication Administration Training through 1/25/2018. Provider's first and and CPR certification will expire 2/2017. Three children's files were reviewed - please see Supplemental Summary for missing items. Description: Based on review of three children's files, the provider failed to have documentation of the signed emergency medical authorization. Evidence: Child A did not have signed authorization for the provider to obtain emergency medical care if needed.

Plan of Correction: Provider will request the signature at today's pick up.
