54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was completed on December 13, 2016. The purpose of this visit was to measure the facility?s compliance with most, but not all, of the licensing standards. At the time of the inspector?s arrival, there were two children in care ages 2 yrs and 9 weeks (6 points) with the substitute provider. The provider returned within 15 minutes of the onset of the inspection. The oldest child has just completed lunch and was being diapered. He was then put down for nap. The infant then awoke and was held while being fed a bottle. The facility administers medication when required and the provider has current MAT certification. However, there was no medication on site. Both providers have current CPR and first aid certification. The last evacuation drill was conducted 12/9/16. The most recent shelter in place drill was conducted 10/12/16. A whole house walk through was conducted and the playground was inspected. Approximate time on site: 11:08am to 1:50pm
Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, the provider did not ensure that the substitute/assistant caregiver's record contained all of the required information. EVIDENCE: 1)The record of the substitute/assistant provider did not contain the name, address and telephone number of a person to contact in the event of an emergency. 2) The record of the substitute provider did not contain verification of education. Plan of Correction: The education information will be placed in the file when received from out of state and the emergency contact information will be completed today.
Standard #: 22VAC40-191-40-D-1-c Description: Based on a review of caregiver records, the provider did not ensure that all background checks were updated within 3 years. EVIDENCE: The record of provider #1 contained a criminal history record check that was dated 10/10/13. The check had not been updated within 3 years of the last criminal history check. Plan of Correction: The criminal history record check will be mailed off tomorrow.
