Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection.

Comments:
An unannounced monitoring inspection was conducted today, December 7, 2016 to determine the provider's compliance with licensing standards. In attendance were 4 children totaling 10 points being supervised by the provider. Children were observed eating snack, playing with toys, and preparing for departure. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All rooms of the home used by children were inspected with no hazards found.

The family day home had the required postings displayed on the bulletin board upon entering the living room. The first aid kit, fire extinguisher, and emergency supplies were inspected. The following was also inspected: fire drill log, shelter-in-place drills, children's records (2), provider record, emergency preparedness plan, parent agreements, and other pertinent information. See attached notice for violations found on this day.

Standard #: 22VAC40-111-800-B
Description: Based on the review of the emergency preparedness plan, the required update was not conducted. Evidence: During the inspection of the emergency preparedness plan it was discovered the emergency preparedness plan was not updated annually as required.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-111-50-A
Description: Based on the inspection of administrative records, the required document has not been maintained. Evidence: During the inspection of administrative records, the required written daily attendance record was not being maintained daily as required.

Plan of Correction: Per provider: I will re-start using the daily attendance form to comply with licensing regulations.
