An unannounced Renewal inspection was conducted in the Family Day Home. The Provider and an Assistant were present. Upon arrival to the home, the LI observed 3 children napping. Based on the ages of the children, today's points totaled 6. The Provider reported 7 children are currently enrolled. After nap, the children were served an afternoon snack of cheese bread and milk. A sample of 4 records was reviewed, 1 child's record and 3 caregivers' records. A discussion was held with the Provider regarding the incomplete Renewal application that was received in the Licensing office. The LI had a blank application and the Provider completed and signed the documents at the time of the inspection. The application was marked as complete on this date. Standard #: 22VAC40-111-830-C
Description: Based on reviewed documentation, shelter-in-place procedures were not practiced a minimum of twice per year. Evidence: The Provider did not have written documentation to verify shelter-in-place drills were conducted.

Plan of Correction: The Provider will conduct drills and will keep a written record at the time shelter-in-place drills are practiced.

Standard #: 22VAC40-111-200-C
Description: Based on documentation reviewed and Provider report, the Provider failed to maintain documentation of the Assistant's orientation. Evidence: The information reviewed for Assistant 2. did not have verification of orientation.

Plan of Correction: The Provider has reviewed all procedures with the Assistant. The orientation will be reviewed again and the form dated and signed.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on reviewed records, the FDH Provider failed to ensure documentation of a criminal history record check verified through the VA State Police was in each record. Evidence: The reviewed record of Assistant #2 did not contain documentation of a verified criminal history record check.

Plan of Correction: The Provider will submit the document immediately to obtain a criminal record background report.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on reviewed documentation, the FDH Provider failed to ensure current criminal history record check reports were updated prior to 3 years since the dates of the most recent criminal record check report. Evidence: The records of Caregivers 1 and 2 did not have documentation of current background reports. The reviewed documentation was dated 07/11/13.

Plan of Correction: The Provider will mail the requests immediately to the VA State Police to ensure background checks are current. Documentation will be maintained in the records.
