The licensing inspector conducted an unannounced renewal inspection from 9:45am - 12:15pm. There were four children in care (one residential) for a total of ten points with one caregiver. The children were observed playing with the available toys, watching a cartoon and working puzzles. One caregiver's, two household member's and three children's records were reviewed. One violation form the previous inspection had been corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-111-100-A Description: Based on record reviews, the family day home failed to obtain documentation of physical examinations within 30 days of attendance. Evidence: 1. The inspector reviewed three children's records. Two of three records reviewed (child record #1 and child record #2) did not contain documentation of physical examinations. The first day of attendance for both children was 11/10/2016. Plan of Correction: The provider has requested copies of the children's physicals, but to date have not received them. The provider will remind the children's parents that copies of the physicals are needed for the children's records.
Standard #: 22VAC40-111-60-B Description: Based on record reviews, the family day home failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed three children's records. Two of three records reviewed (child record #1 and #2) had two emergency contacts, but the address for one contact was not completed. Plan of Correction: The provider has already requested this information, but will ask the parents for this information again today. The provider will review the children's records to make sure they contact all required information.
Standard #: 22VAC40-111-80-A Description: Based on record reviews, the family day home failed to obtain proof of child's age and identity within seven business day from child's first day of attendance. Evidence: 1. The inspector reviewed three children's records. Two of three records reviewed (child record #1 and child record #2) did not contain proof of child's age and identity. The first day of attendance for both children was 11/10/2016. Plan of Correction: The provider has requested this information from the children's parents, but will request this again today at pick up time.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record reviews, the family day home failed to obtain repeat background checks before three years since the date of the most recent central registry finding. Evidence: 1. The inspector reviewed one caregiver's and three household member's records. All four records contained searches of the central registry that were over three years old. Caregiver record #1 and household member record #1 contained searches of the central registry that were dated 10/24/2013. Household member record #2 and #3 contained searches of the central registry that were dated 11/11/2013. Plan of Correction: The provider requested repeat central registry checks in November, but to date they have not been received. The provider will call and inquire about the status of the requests. The provider will let the inspector know when the background checks are returned.
